Position: Essex County Family Justice Center (ECFJC) Health Navigator
DESCRIPTION: The ECFJC Health Navigator will conduct screenings, assessments, referrals and follow up for medical, mental health and substance abuse services. This position reports directly to the ECFJC Program Coordinator.
This is a full time (37.5 hours per week) position.
RESPONSIBILITIES INCLUDE: Conduct comprehensive intake with clients Assess for medical, mental health and substances abuse conditions Facilitate referrals to appropriate medical, mental health and substance abuse services Ensure that clients are receiving eligible entitlement benefits such as Medicaid. Maintain accurate and confidential records Perform other duties as deemed necessary by the ECFJC Executive Director
QUALIFICATIONS: Bachelor’s Degree in social work required, Master’s preferred Bilingual Spanish or Portuguese required Knowledge and experience in domestic violence field Knowledge of the medical, mental health and substance abuse systems Knowledge of the Welfare System Completion of 40 hours domestic violence course preferred. Ability to work independently as well as collaboratively Strong organizational, writing and interpersonal skills.
Knowledge of computer programs including but not limited to Microsoft Office and PowerPoint
